Preparing for your online assessment: what to have to hand

Health & Wellbeing

A short checklist of the details worth gathering before you start, so the assessment takes minutes rather than a return visit.

The online assessment is short, but it goes fastest when you have a few details ready. Here is what we suggest gathering before you start.

Your current medicines

Have the names of any medicines you take to hand, including anything bought over the counter and any supplements. The packaging or a repeat prescription slip is the easiest place to find the exact names.

Allergies and existing conditions

You will be asked about known allergies and any conditions you have been diagnosed with. If you are unsure of a name, describe it in your own words; the prescriber can ask a follow-up question if needed.

Height and weight

Recent measurements are best. Approximate figures are fine if you do not have exact ones.

Your GP surgery details

We ask for your GP surgery so that, with your consent, we can let them know about treatment you receive through our service. The surgery name and postcode are enough.

Somewhere private

The assessment asks personal questions. Choose a moment when you can answer honestly without interruption. Your answers are saved as you go, so you can always stop and return later.
